Many older kickoff-time moms face a dilemma when it comes to baby No. two. The clock is ticking louder than ever. But doctors suggest waiting at least a yr and a one-half after giving birth earlier conceiving again.

This is the standard advice, based on multiple studies and public health guidelines. But deciding when to try again tin can be a difficult decision — weighing medical risk against infertility risk. Now in that location are some new data points to factor in. A paper published Monday in the journal JAMA Internal Medicine analyzed medical records from most 150,000 Canadian pregnancies to tease out how a mother'due south historic period influences the effects of a shorter-than-recommended interval between pregnancies.

For older moms in a bustle, the bad news is that the written report adds show that conceiving within 12 months of a nascency does mean heightened health risks for both mother and child. Just epidemiologist Laura Schummers, who led the research while at Harvard and is now a mail-doctoral swain at the Academy of British Columbia, says there's skilful news for you lot here as well:

"The optimal spacing window that we establish was one to two years after the commitment of one kid until the conception of the next pregnancy," she says. "That'south when we found the everyman hazard for both mothers and babies." And, she adds, that's short compared to some previous studies that had suggested the optimal await was between 18 months and upward to five years.

Past research has found a clear link between brusk "interpregnancy intervals" and increased risk of health issues for female parent and baby, including premature birth. But why? The debate, Schummers says, revolves around whether the brusque interval is a direct biological cause of the risks, or whether information technology it is itself a effect of other forces at work in the mother's life — for example, a lack of access to health intendance and unintended pregnancies.

Because older women are likelier to plan their pregnancies and have better access to care, Schummers and colleagues hypothesized that those mothers would non incur as much risk every bit younger women do if they had babies close together.

They establish out they were wrong.

"In fact," Schummers says, "we institute that there were risks of agin baby outcomes for women of all ages.

"The risks to the babies were higher among younger women, which was consequent with the team's hypothesis. But risks to the mothers were college among older women — indeed, only older mothers incurred higher risks to their own health by getting pregnant once again so soon.

After accounting for other factors that could drive these numbers, Schummers says, the stats milk shake out like this:

• For women 35 years or older who conceived just 6 months after a nativity, vi.2 per thousand experienced serious illness or injury, including decease. Expect xviii months and that hazard dropped to 2.half-dozen per per thou. So, small-scale accented numbers simply a dramatic difference.

• A "astringent adverse babe outcome" includes stillbirth and being born very early on or very minor. Amidst women ages 20 to 34, those who conceived after only six months had 20 babies per grand with those astringent outcomes; the risk drops to 14 per thousand amidst those who waited 18 months.

• Among women 35 years or older, in that location were 21 astringent infant outcomes per thousand amongst those who waited simply vi months; the risk drops to 18 per thousand amongst those who waited 18 months.

"This shows you both the relationship between pregnancy spacing and the increased risk," Schummers says, "simply also that older women tend to have a higher baseline adventure of many of these outcomes at all pregnancy spacing lengths."

The research turned upward a like pattern for premature nascency: A short pregnancy interval raises the risk for all women, but particularly for younger women. The risk for them dropped from 53 per g at a 6-month interval to 32 per thousand at an 18-month interval. For women over 35, the risk dropped from 50 per thousand at six months to 36 per thousand afterwards 18 months.

Information technology seems similar common sense that a adult female's body may need more than six months to fully recover from building a baby and giving nascency, only the actual mechanism behind the risks of curt pregnancy intervals is not fully clear.

The leading theory, Schummers says, is that nutrients similar fe or folate could be depleted in the mother's torso. But more research is needed to see if that theory holds in adult countries like the Usa and Canada, or if there are other mechanisms that take not still been identified.

For now, she says, her team hopes these new findings tin can aid women brand decisions within their own personal contexts, and in consultation with their medical teams. The data may be particularly helpful for older women, she says, considering they more often make up one's mind to have short pregnancy intervals on purpose.

"And then if you're making that kind of decision on purpose," she says, "it's easier to say, 'You know, let's wait some other three months.' "
